Does Aetna Cover a Gym Membership?
Last checked October 13, 2026
Depends on the plan. Every individual Aetna Medicare Advantage plan includes a SilverSneakers basic gym membership at no additional cost, and some of those plans also reimburse fitness fees up to a yearly amount ($600 and $1,200 are figures in Aetna's own 2025 plan documents). On an employer plan the only gym benefit is a fitness reimbursement the employer chose to add, with the amount in your benefits booklet. No Aetna plan pays for a gym as medical care.
Does Aetna cover a gym membership?
Not as a medical benefit, and the answer depends on which Aetna plan you hold. On Aetna's individual Medicare Advantage plans, yes in practice: every plan includes a SilverSneakers basic fitness membership at no additional cost. On employer plans, sometimes: Aetna lists fitness reimbursement among the extras an employer can add, and the amount is the employer's choice. On an individual commercial plan bought on an exchange, usually nothing beyond discount programs.
Aetna Medicare Advantage: SilverSneakers at no extra cost
Aetna's own 2026 plan announcement states it: a SilverSneakers basic fitness membership for all individual Medicare Advantage members at no additional cost, with access to thousands of gyms, exercise programs at community centers and parks, and nutrition and well-being classes. The one exclusion Aetna names is its Institutional Special Needs Plans (I-SNP), whose members are in care facilities.
A basic SilverSneakers membership means facility access, standard equipment and SilverSneakers classes at locations that have a SilverSneakers contract; premium services at a gym (personal training, some class programs) are not included, and a gym without a contract takes nothing. Confirm eligibility on the SilverSneakers site to get a member ID, then use its location finder rather than assuming your usual gym participates.
Aetna Medicare fitness reimbursement
Some Aetna Medicare Advantage plans add a second benefit: a fitness reimbursement that pays you back for certain fitness expenses up to a yearly amount. Aetna publishes a dedicated reimbursement form for it each year, covering aerobic and fitness activity fees, membership fees, activity fees and fitness supplies, with receipts due within 60 days of the end of the quarter you paid in. The amount is set per plan: 2025 plan documents show $600 a year on one plan and $1,200 a year on another, and plans without the benefit show nothing.
To know yours, open the plan's Summary of Benefits or Annual Notice of Changes and search for "fitness reimbursement". If it is there, keep every receipt from day one; a claim filed late in the year for the whole year will miss the quarterly deadline.
Aetna employer and individual plans
On commercial plans the benefit, where it exists, is a reimbursement the employer chose to include, so Aetna's public pages describe it only as an available extra ("health screenings, mental health support, fitness reimbursement and more") without an amount. Typical designs pay a fixed sum per year once you meet a visit minimum, claimed through the member portal with proof of membership and visits. Your benefits booklet has the number; if it isn't there, your employer didn't buy it.
Aetna members also get discount programs on fitness products and memberships through the member site. Those are price cuts, not payments, so compare the discounted rate with the gym's own promotions before joining through the program.
Whatever your plan does, two routes don't depend on it: a Letter of Medical Necessity can make the dues a qualified expense for a health savings account or a flexible spending account, and what the gym itself charges is the number any reimbursement is measured against.
